- Documentation of the name of the original creditor and the current creditor or owner of your debt.
- Your last account number with the original creditor.
- A copy of the signed contract, signed application, or other documents providing evidence of your liability and its terms.
- The date that your debt was incurred.
- The date of your last payment, if applicable.
- An itemized accounting of the amount claimed to be owed, including the amount of the principal, any interest, fees, or charges, and whether the charges were imposed by the original creditor, a debt collector, or a subsequent owner of the debt.

Notice of Important Rights: You have the right to make a written or oral request that telephone calls regarding your debt not be made to you at your place of employment. Any oral request will be valid for only ten days unless you provide written confirmation of the request postmarked or delivered within seven days of such request. NEW YORK STATE RESIDENTS:
